To register via U.S. Mail, download the appropriate form below. Complete the form out (fields are fillable in Adobe Acrobat Reader), print, sign the Waiver/Release Form and enclose all pages with a check for $90 payable to the Upstate Outdoor Lacrosse League. Mail registrations to:
